fix: all 7 GraphQL queries — correct field names and LINKS/PHONES subfields

Leads: landingPage → { primaryLinkUrl }, remove duplicateOfLeadId
Ads: adFormat → format, destinationUrl/previewUrl → { primaryLinkUrl }
FollowUps: remove non-existent callId field
LeadActivities: durationSeconds → durationSec
All transforms updated to match query changes
